rules work with body too, for sure. however i'm not sure if HMS first decodes base64 and then performs a search, like SA or ASSP do it. i think not. so only text and/or some HTML bodies are good for rules. depending on traffic volume and average message size it may not be wise to use body rules thou...

try GLOBAL rules, Set Header Value action, i.e. whoever replies to customer, sends out support@company.com as From. so, thread goes on between support group and customer.

but don't rely on ClamAV alone. to me, without Sanesecurity it's almost useless.
a good solution is Spamassassin. trap any extension and score.

Seriously, in hmailadmin there is a button called 'Empty Folder'. How hard can it be to press it? IMO the problem is millions of files on system. clicking on "Empty Folder" for sure will start a process, but i suspect when and how it ends - if ever. millions of messages... i don't know about your hardware but it will take ages to remove them with clean methods. very likely your entire system will hang. so, a very dirty suggestion if that account has a reasonable amount of messages other than those in trash: copy messages to a temporary account....

there is a good script here deleting x-days old mails in certain IMAP folders.
